Trick or Suite

The Embassy Suites of Piscataway and Coordinated Family Care invite CMO involved youth and their families to attend, “Trick or Suite”, on October 29th, 2009 from 5-7pm at the Embassy Suites of Piscataway located at 121 Centennial Ave. Piscataway, NJ.
To attend this event please get in contact with your Care Manager at (732) 572-3663.
Each participant will have the opportunity to trick or treat for candy and goodies throughout the decorated suites in the hotel. It is suggested that all participants wear a costume.
Please note - No youth will be admitted without an adult chaperone. 35 slots are available for this event, first come first served basis.

Celebrating Our Youth
|
As a recent “Graduate” of CFC, along with Billy his entire Child and Family Team members have a lot to celebrate. After being enrolled in CFC for almost 4 years Billy was recently discharged from the CMO and is now looking towards his very bright future. When asked about who Billy credits for all of his success he openly replies with, “ I would not be where I am today if it was not for my Care Manager Kelly Nasert—who always gave my team great ideas about how to help me get better but also my family for sure! My family has always stuck by me and I have learned to calm down and listen to them before I react.”
After receiving his High School diploma in 2008 he has been able to his credit become a junior volunteer Fire Fighter, held two jobs, and volunteers for the POP Warner Football league in his town. In his own words Billy’s plans also include, “being off medication in a few months and becoming a Navy Fire Fighter.” Billy has also volunteered his time to CFC in making committee decisions not only with the Service Provider Wraparound Training itself but the Youth Event Committee as well.

“The Ohana Luau 2009” The Family Party
The Amazing Ohana Luau Youth Event on June 26th, 2009 was a success!! The event provided CFC youth with an opportunity to get together and enjoy a day filled with exciting activities that helped enhance their personal growth, promote team work and a positive self image. This event would not have been possible without the commitment and dedication of the CFC youth who participated on the planning committee for this event. Their creativeness and ability to give back to their peers is admirable and appreciated! CFC youth also assisted with the facilitation of the days activities along with setting up, and cleaning at the conclusion of the day. We already have a handful of possible youth committee members willing and ready to go, for next’s years youth event. Thank you again to our youth – we are listening!

CFC Annual Provider Wrap Around Training
Coordinated Family Care will be holding its’ provider wrap around training on 5/15/09. Registration forms for this training have been sent to providers via mail. The training will be held at our office located at 30 Silverline Dr., Suite 1, North Brunswick, NJ 08902 from 12-3:30pm, A light lunch will be served from 12-12:30pm.
This session fulfills the wrap around training obligation providers have in their agreements with CFC and all direct staff are encouraged to attend. CFC also encourages the presence of at least one member of management from each provider agency.
If you have not received a registration form in the mail, are a provider agency in Middlesex County and are interested in attending, you may access the form here.

WHAT works for me may work for you……….
As a CFC youth for over a year I have really learned some great things about myself along with my abilities. If you find yourself in a difficult situation try the following:
Don’t take things personal, everyone has a bad day!
Talk to someone you trust and don’t hold your feelings inside, you may end up going over the edge.
Surround yourself with positive people– my BA , friends & family are my support!
Hang in there, every experience is a learning opportunity!

Coordinated Family Care, Middlesex County Care Management Organization (CMO), as part of New Jersey’s Division of Child Behavioral Health Services, is committed to partnering with existing community providers and also facilitating the expansion and development of additional resources to meet the needs of children and families.
Coordinated Family Care announces the availability of funds specifically designated to aid community based agencies to create needed services. Applications will be accepted from service providers who currently have an Agreement with Coordinated Family Care or are interested in initiating and completing an Agreement with Coordinated Family Care. Coordinated Family Care will provide the guidelines for application.

Back to School!

As CFC youth prepare to return back to school, the United Way of Central Jersey lends CFC a hand. For the upcoming academic year 2008-2009 CFC is expected to receive school back pack donations in all sizes and colors.
The back packs will be cool enough for our teens through young adults who are returning to school as well as our kindergartners who are starting school for the first time. Not only will we receive back packs but some great school supplies as well.
These donations are set aside for CFC families and youth who are in need of assistance. If you are a service provider and would like to add to this donation please contact the Resource Department. If you are a CFC family in need please contact your Care Manager directly. Together we can make this school year a great experience for everyone involved. Learning can be fun! Thank you UWCJ!

CFC Youth Event Update
|
The "Amazing Race” CFC youth event
was a success from beginning to end. On June 27th, 2008 over 40 of
our youth enjoyed a fun filled day of team building and recreational activities.
From the great hula hoop contest, to the word scramble game all the way to the
boat building contest, everyone was a winner!
The day would not have been possible
without all the hard work and creativity of CFC staff and our community
partners.
The Elks Club of Old Bridge deserve
a very special acknowledgement for their donation of space and food as well as
their time and willingness to assist in anyway possible.
Joe Parauda, owner and operator of Chef In Thyme also
donated his invaluable services of cooking lunch during the event along with two
great side salads.
A final thank you to our priceless
service provider volunteers from The Youth Success Network, the Youth Advocate
Program and the Family
Support Organization of Middlesex County – we could
have not done it without you all!
Our next youth event is scheduled for this
October, revisit our website for updates as there will be more to come!

Join us for an “Amazing Race”……

On June 27th, 2008 CFC youth, employees, local service providers, the Middlesex County Youth Partnership and the Elks Club of Old Bridge NJ will participate in a fun filled day for youth enrolled in our agency and their siblings.
The theme for this year’s event is “The Amazing Race” and we look forward to seeing our youth compete in a series of creative and challenging activities. The event would not be complete without food and prizes, so come hungry and ready to win!
