Coordinated Family Care (CFC) is a private non-profit Care Management Organization funded by the NJ Department of Children and Families. We provide guidance, options and assistance to Middlesex County families that care for youth who present with very high levels of behavioral and/or emotional needs.
Care Management Organizations are an important part of a state-wide effort to improve New Jersey’s mental health system of care for youth and their families.
CFC’s approach to helping families is grounded in a philosophy that begins with three key elements:
Choice
To provide the right services to families at the right time, CFC has developed relationships with Mental Health agencies and other types of providers throughout the county. We make an effort to learn about each service provided by an agency so we can provide families and youth with informed and objective guidance as to the options available to them.
Voice
CFC works with the family and whoever the family wants involved (aunts/uncles, neighbors, clergy etc) to develop a plan of care. Professional intervention is seen as a temporary resource for this “Child Family Team”, as the premise of our work is that families can remain together with a majority of community based supports and resources in place.
Resources
We assist families access creative, non-traditional services. CFC will also help families apply for and obtain state benefits that will assist with longer term mental health care.
These three elements are keys to the wrap around approach which is being used nationwide. This approach has consistently produced improvements in the lives of youth, families and communities.
Coordinated Family Care is not a treatment program, we help families create, develop and access opportunities to meet needs.
